In 2008, I had to have emergency surgery to remove a peri-rectal abcess the size of a grapefruit. After surgery, I had to have a drain tube in for 6 weeks. Once it was removed, I was left with a small scar on my right butt cheek.
Fast forward to now: My caregivers have noticed that this scar site has broken down several times in the past 2 years. I have been doing pressure relieving techniques throughout the day (push-ups in my wheelchair, a new wheelchair cushion, etc.). We even tried keep a patch of DuoDerm on the site constantly, but it didn't help much. All the DuoDerm did was caused my bottom to itch constantly.
Right now, the site doesn't have skin breakdown, but it is starting to get that telltale itching and a rash over it. The same thing happens everytime it starts to break down. Is there anything I can do to prevent that scar from breaking down constantly? It seems like once we get it healed up, a couple weeks later, its broken down again!

This may help you :take one capsule of the herb, Golden Seal, open it and put it into a small bowl, then take a gel capsule of Vitamin E, poke it with a straight pin, squeeze it, and put it in with the Golden Seal, and mix the two together. Then cover the sore. Now you want to alternate this with Aloe Vera Gel. I got Cellulitis many years ago and could not get the sore to stay healed, it kept on opening. But this remedy healed it and strengthened the skin somehow. I will also give you my cure for pressure sores. One that I have found that has saved my Butt (literally) on more then one occasion. It is an Amish cure, and believe it or not it is Preparation H, but you want to use the ointment not the cream. Because they have changed the ingredients in the cream, (use the one with fish oil in it) this is good for a pressure sore that is not already opened. You will be amazed at the results when you use the stuff!! I have no idea if it would heal an open pressure sore or not, because I have never tried it. There is a cure that works really well for a pressure sore that is opened. The 2 that I got were before I found the cures, and they were extremely small.

I've had dressings like duoderm and others CAUSE a rash which in one case broke down. A cortisone and anti fungal stopped it. See if the rash is the same shape as the dressing.

A friend (C6-7 quad) said that his podiatrist suggested Amerigel for a persistent pressure sore on his foot and that it worked well. I don't know how well it would work on other areas but it might be worth a try. It is available from the manufacturer, Amazon, and ebay as well as many retail outlets.
