( i posted this at carecure, but figured i might as well post it here to see if any of you can help or come out with some other ideas)
...around my rectal area this has been going on for like 2 weeks and it is so painful and discomfortable
hi, all,
i wear diapers and move my bowels around 3/4 am every morning. i am done around 5:30 when my beloved ma comes to clean me up. we never had problems like that.
the area is in the crack, between the two cheeks surrounding the rectal area. it is very irritated, red and with raw spots.
we have tried hydrocortisone, lanacane, and my doctor prescribed something called epi foam that i have started today.
any ideas on how to make this better?
it hurts so much.....

Try one of the many skin protectors like skin prep. Then try getting a gauze pad and sticking it between to absorb moisture. If it helps you will know the area wants to be dry.
Also, the pH of your feces may affect it. It's most likely acid if anything so washing with baking soda or hydrogen peroxide might help.

thank you for replying and for the ideas, paul
i forgot to mention that my bowels are very spastic, so they irritate my bladder, making it leak, even though i have had botox injections.
every time i have a contraction to go, i leak through my urethra and i supoose that irritates the raw skin, too, and washes out everything i have put in it to prevent the poo from having contact with the raw skin......
i don't know what else to do....... i have thought about a colostomy.
anyone else has any input?

Lola, if you haven't tried Desitin yet, you should try it.
Have your aide gently clean the irritated area with lukewarm water, then dry it with a soft tissue, by patting gently, never rubbing, because rubbing will irritate the skin more and make the problem worse.
Then let her apply a good, thick coating of Desitin to cover the irritated area completely.
Desitin contains natural oils that are very thick, so it will not get washed away by urine or feces, and it will protect your skin from irritation very effectively.
It also has vitamins A and D, and zinc, that will promote healing.
I hope it helps.

thank you for replying.
desitin has helped me a lot when the rash has been on my cheek (butt) but somehow it is not adhering so well on the raw skin.... 
i love desitin, it is the first cream that my ma applies when i am getting a rash.
this raw skin is being a pain in the ass, literally!

Have you changed laundry detergents or has the one you use been "reformulated?" If so, trying another might help. Try something mild like Ivory.

I am so sorry you have to go through all of this. I do not understand why people have to suffer, and it always seems to be the ones that are of the top people. Honey, if I could take away your pain, I would. You are a beautiful person and I am blessed to have come across someone like you, never mind the friendship that you have given me. I'm sorry to hear that your skin is so raw that desitin does not adhere to it. But maybe another product, called Cavilon No-Sting Barrier Film could help you.
It's alcohol free, so it won't sting or damage sensitive tissues, and it's made especially to protect the skin of incontinent patients from damage due to urine or feces, and it can be applied even to raw or denuded skin. It is recommended for daily use by patients who wear diapers to prevent skin damage.
It comes in a small pump-spray bottle, and you spray it on the skin, starting around the anus and moving outward, and when it dries, it forms a waterproof protective film that lasts for at least a day.
They also make a Cavilon Skin Cleanser which is less irritating than soap and water, so it does not need to be rinsed off, and is recommended for routine skin cleaning of the diaper area.

But, unfortunately, if your skin is very bad, and oozing a lot of fluid, then the Cavilon might not be able to adhere too well to the oozing areas, but it would still help protect the other areas that are not oozing, and keep them from getting worse.
Also, skin infections with Candida (yeast), or with bacteria, are very common in diaper dermatitis, and can make the problem worse, so you should ask your doctor if she thinks that this could have caused your problem to get worse in the last few weeks, because if you didn't have this type of painful irritation before, then something must have changed recently to cause it.
And if you don't get better in the next two weeks, you should consider seeing a wound care specialist, because if the skin on your butt continues to get worse, you could develop serious pressure ulcers, and you don't want that to happen.

I agree. Check out the pictures on the bottom of this page. They're pretty gross, but on the bottom, in the middle, is a picture that may represent what is happening to you. Have your ma look at them and see if this might be going on with you.
